Safety incidents generate sets of recommendations. However, the implementation of these ‘lessons learned’ is often unsuccessful and the incident recurs. This project aims to better understand why this is the case by identifying those factors most likely to ensure safety in complex systems and which of these are most easily adopted. To build these fundamental insights into learning from failures in complex systems we will deploy a modified qualitative Delphi method in a series of online workshops, which will lead to a report for the Royal Academy of Engineering, our project sponsor.
